The Boys & Girls Clubs of Utah County announces the sponsorship of the Summer Food Service Program. Free meals will be made available to attending children.
In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, religious creed, disability, age, political beliefs, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. Meals will be served by the Boys & Girls Clubs of Utah County from 11:30am-1:00pm M-F from the dates of June 1, 2018 – August 24, 2018 at the Provo Clubhouse located at 1060 E. 150 N. Provo, UT 84606. Meals will also be served at UVU’s Student Health and Wellness Center (800 W University Pkwy, Orem, UT 84058) from 11:00am-12:30am M-F from the dates of June 4, 2018 – August 24, 2018
